CERTIFICATION OF APPLICATION:

Please read the following statements carefully and check the boxes as confirmation of your having read and accepted these conditions of employment.
  1. I understand that this electronic application is my complete application, and that I am to apply online for jobs. This duty includes the need to self-report any arrest(s) or charge(s) incurred following the submission of my application. I understand that I should not send materials to individual hiring managers or principals. If any materials are sent to hiring managers or principals (including emails, resumes, etc.), the hiring managers or principals are under no obligation to respond to them or keep them on file.
  2. I hereby certify that, to the best of my knowledge, all information provided by me on this employment application and all other information provided by me in the course of applying for employment with Charlotte - Mecklenburg Schools is accurate and complete.
  3. All material submitted with this application becomes the property of Charlotte - Mecklenburg Schools 
  4. All employees, contracted and substitutes, must present the results of a negative tuberculin test taken within 2 weeks after orientation (or approval for substitutes) and provide documentation to verify employment eligibility.
  5. I authorize Charlotte - Mecklenburg Schools to conduct a background investigation and authorize release of information in connection with this application for employment. This investigation may include such information as criminal convictions, driving records, previous employers and educational institutions, personal references and other sources deemed appropriate in the sole discretion of Charlotte - Mecklenburg Schools. I waive my right of access to any such information, and without limitation hereby release Charlotte - Mecklenburg Schools and the reference source from any liability in connection with its release or use. This release includes the sources cited above and illustrative examples as follows: the local Sheriff, information from the Central Criminal Records Exchange of either data on all criminal convictions or certification that no data on criminal convictions are maintained, information from the North Carolina or other state Department of Social Services Child Protective Services Unit and any Locality to which they may refer for release of information pertaining to any finds of child abuse or neglect investigations involving me.
  6. Copies of transcripts of all college work must be submitted with this application to the Department of Human Resources. Official transcripts mailed directly from colleges/universities will be required once employment is offered.
  7. I unconditionally certify that I have made true, correct and complete answers and statements on this application in the knowledge that they may be relied upon in considering my application. I acknowledge that these questions shall be continuing in nature, and I have a duty to update, change or furthermore amplify my answers to guarantee accuracy at all times. I understand that any omission, misleading or falsely answered statement made or implied by me on this application, or any supplement to it, whether written or oral, will be sufficient grounds for failure to employ or for my immediate discharge should I become employed with Charlotte - Mecklenburg Schools. In the event the School Board determines, in its sole discretion, the existence of a material adverse report or omission as to any information, I agree that the employment offer/appointment will be deemed revoked immediately without further action, notice, or process. In conclusion, I acknowledge that if accepted by Charlotte - Mecklenburg Schools for employment, I hereby agree to abide by the policies, regulations, and directives of the school division.
  8. I hereby authorize the release of any information relating to my current and previous employment, education, personal history records, military service records, and criminal history records. I fully waive any rights or claims I have against the organization(s), its employees, representatives, and agents providing such information from any and all liability claims or damages that may directly or indirectly result from the use, disclosure, release or omission of any such information by any person or party.
  9. I understand that, if employed in a position that requires driving as one of the functions, it is my duty to immediately advise my supervisor in writing of all traffic accidents and/or driving violation citations I have received or may receive in the future. Furthermore, it is my duty to immediately advise my supervisor in writing of changes in my health condition that may present a threat of harm to my own health or safety, or to the health and safety of others. Failure to comply may be deemed just cause for termination by the Charlotte - Mecklenburg Schools. I further understand that Charlotte - Mecklenburg Schools has a vital interest in maintaining safe, healthful, and efficient working conditions for its students, the public, and vehicle operators. Using or being under the influence of alcohol and/or drugs on the job is illegal and may pose serious safety and health risks not only for the user, but to all those who come in contact with the user. The possession, use or sale of alcohol or an illegal drug poses unacceptable risks to safe, healthful, and efficient operations.
    • As a CDL driver applicant, I hereby understand and agree to submit to pre-employment alcohol and drug testing, pursuant to the Code of Federal Regulations (49 CFR 382), which requires CDL drivers who wish to be considered for employment to submit to pre-employment alcohol and drug testing.
    • I understand that any CDL driver applicant who is unwilling to agree to these conditions should not apply to Charlotte - Mecklenburg Schools.
    • I understand that for CDL driver applicants, a positive alcohol and/or drug test, or a refusal to test, will result in the applicant being removed from consideration or a withdrawal of an offer of employment, if an offer has been made.
    • School bus drivers and other specific positions that require a Commercial Driver License with a S endorsement are required to hold a DOT medical.
  10. I further understand and agree that: 
    • My application will become inactive in 12 months. Should I wish to be considered for employment after that, I must reactivate my application by logging in.
